You will receive and email and text message confirming your appointment. Please follow the directions to complete all of your new patient forms before your first visit.

Please bring the following things to our Jacksonville, FL location when you arrive for the first time:​

• Your insurance cards and/or claim forms

• Your referral information (if applicable)

• A list of any current prescriptions

• Any relevant medical/dental history

• Any questions you have!
